Privacy Policy


EHIM is unequivocally committed to securing and protecting your confidential information including any personal and medical information. Upon enrollment in any EHIM program or service, we ask only for the minimal set of information required to fulfill your needs at the level of service you have come to expect from EHIM. It is understood that when enrolling in an EHIM program or service, all information provided is done so on a voluntary basis. However, failure to provide necessary information could result in EHIM being unable to process your claims.


Collection of Personal Information


When you register for EHIM products or services, you may voluntarily provide EHIM with personal information such as your name, mailing address, email address and other information. This registration may occur by mail, through your employer, through our web-site or through other channels.


As a beneficiary of EHIM's Prescription Benefits Management, Insurance or TPA products, EHIM may collect your health-related information for the purpose of processing your prescription claims and fulfilling the needs of your selected service(s).


Our privacy policy and practices are described in the notice under the Federal Health Insurance Portability and Accountability Act ("HIPAA").

EHIM reserves the right to aggregate your personal, health-related data. Aggregating data refers to the process of summarizing and performing statistical analysis on data. The aggregation of data removes your personal identifying information from the data so that the data in no way identifies you.


In order to process health and prescription claims through the plan, EHIM is provided specific health-related information. The healthcare provider secures the patient’s consent to submit the minimum necessary claim information for payment to be issued. Claims are either sent in paper form by mail or sent electronically and may include the following patient information:


  • Name
  • Address
  • Member Identifier
  • Diagnosis
  • Service provided (i.e.: office visit and exam, labs, x-rays, surgery, etc.)
  • Medications dispensed


Occasionally, claim processing requires more information than what is initially provided. EHIM will request only the minimum necessary to make a benefit determination. Some situations which may require obtaining additional information would be pre-authorization of a proposed medication or confirmation of payment.


Collection of Non-Personal Information About Your Computer


When your browser loads a web-page from an EHIM web-site, EHIM may record technical information including the type of browser you are using (e.g. Microsoft Internet Explorer 10 or FireFox), the operating system you are using (i.e. Microsoft Windows 7) or the kind of computer you are using (i.e. Macintosh). We collect this information for the purpose of improving the quality of our web-site and web-services. We will not sell or disclose this non-personal information to other companies or any other organizations.

Children's Online Privacy Protection


EHIM's websites are neither intended nor designed for use by minors. EHIM has no intention of collecting personal information from minor children.

To request the removal of personal information submitted by a minor child, please send an email.


Email and Website Privacy


Email may be sent to EHIM representatives by following email links found on the EHIM websites. EHIM representatives may respond in email unless instructed by the sender to do otherwise.


Standard email is not a secure form of communication and should not be used to communicate personal information such as medical data, financial data or social security numbers. Email may be intercepted by third parties. By using the e-mail service, you accept the risk of transmitting information from your e-mail address to EHIM in an unsecure environment.


If you are receiving e-mail correspondence at an employer's e-mail address, please note , under law the employer may be permitted to view the contents of any e-mail messages received at this address. EHIM will not share your email address with any other company without your permission.
