Collection of Personal Information
When you register for EHIM Services, you may voluntarily provide EHIM with personal information such as your name, mailing address, email address and other information. This registration may occur by mail, through your employer, through our web-site or mobile app or through other channels.
EHIM may collect your health-related information for the purpose of processing your claims or otherwise fulfilling the needs of your selected Services.
EHIM reserves the right to aggregate your personal, health-related data. Aggregating data refers to the process of summarizing and performing statistical analysis on data. The aggregation of data removes your personal identifying information from the data so that the data in no way identifies you.
In order to process claims, EHIM is provided specific health-related information from healthcare providers or payors as the case may be (the, “Provider”). The Provider secures your consent to submit the minimum necessary information for payment to be issued. Claims may be submitted in paper form by mail or sent electronically and may include, but are not limited to the following member information:
- Member Identifier
- Service provided (i.e.: office visit and exam, labs, x-rays, surgery, etc.)
- Medications dispensed
Occasionally, claims processing requires more information than what is initially provided. In this instance, EHIM will request only the minimum necessary amount of additional information to process the claim(s).
Certain information which may be inputted by you into EHIM applications or websites could be protected health information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). EHIM MAKES NO WARRANTY OR REPRESENTATION THAT THE DISCLOSURES OF INFORMATION BY YOU OR ANY HEALTHCARE PROFESSIONAL TO EHIM ARE PERMISSIBLE UNDER HIPAA AND/OR THE PRIVACY AND SECURITY RULES. YOU AND EACH HEALTHCARE PROFESSIONAL ARE SOLELY RESPONSIBLE FOR ALL DECISIONS YOU MAKE REGARDING THE USE, DISCLOSURE OR SAFEGUARDING OF “PROTECTED HEALTH INFORMATION.”

Technical and legal circumstances beyond our control could prevent EHIM from ensuring that your information will never be disclosed in ways not otherwise described herein. For example, among other things, we may be required by law, regulation or court order to disclose information to government representatives or third parties under certain circumstances. If EHIM is requested by law enforcement officials or judicial authorities to provide information on individuals, EHIM may, without your consent, provide such information. In matters involving claims of personal or public safety or in litigation where the data is pertinent, EHIM may use or disclose your personal information without your consent or court process. Unauthorized parties may unlawfully intercept or access transmissions despite any commercially reasonable security efforts by EHIM. Even with such technology, no website or application is 100% secure. Therefore, EHIM does not promise, and you should not expect, that your private information shall remain private under all circumstances and you shall not hold EHIM or its business associates liable for its failure to do so.
Collection of Non-Personal Information About Your Computer
When your browser loads a web-page from an EHIM web-site or application, EHIM may record technical information including the type of browser you are using (e.g. Microsoft Internet Explorer 10 or FireFox), the operating system you are using (i.e. Microsoft Windows 7) or the kind of computer or device you are using (i.e. Macintosh). We collect this information for the purpose of improving the quality of our web-site,web-services and applications. We will not sell or disclose this non-personal information to other companies or any other organizations.

Children's Online Privacy Protection
EHIM's websites and applications are neither intended nor designed for use by minors. EHIM has no intention of collecting personal information from minor children.
Email, Website and Application Privacy
Communication that you send to EHIM via the email links and forms on our site or application may be shared with a customer service representative, employee, or medical expert that is most able to address your inquiry.
Because email does not provide a completely secure and confidential means of communication, please do not use e-mail if you wish to keep your communication private, instead, call 800-331-3446 with your request.
EHIM will not share your email address with any other company without your permission.
